New Patient Registration
To register at this practice you will be required to complete the necessary registration paperwork provided by the Practice. You will be required to bring proof of I.D, proof of address, details of your previous GP and NHS number if you have it. If you have your medical card, please bring that with you.
If you have not been registered with a GP previously please bring I.D. for all family members i.e. Passport, I.D. Card, Letter/Utility Bill (dated in the last 3 months) with person's name and current address.
All new patients registering at Langley Health Centre will need to have a New Patient Health Check. This check is carried out by a Health Care Professional - this health check must be carried out to complete the registration process. All new registrants over the age of 16 will be offered screening for Hep B/C and HIV.
We register patients between 9.30am - 10.30am and 3.00pm - 4.00pm Monday to Friday.
If you book an appointment for your health check you must cancel the appointment if you are unable to attend. Please ring 01753 544288 to cancel your appointment. Please cancel the appointment 24 hours in advance so that it can be offered to another patient.
If you need to see a doctor before you are registered, or before the registration process has been completed, we will see you either as an "immediate necessary" or "temporary resident".
Changes to Personal Details
If your personal details change, please inform the receptionists who will be pleased to update our records.
